1. This is SMS not Game Gear!
Yes this has .sms extension instead of .gg and that is because a handful of Game Gear games were simply the Master System version in a Game Gear cartridge.
Usually a GG emulator will play SMS roms, since they are practically the same hardware.
.sms extension is needed for this game to work properly, renaming might cause issues (or not) depending on the emulator. You'll have to live with this.
2. Why not just skip this and upload the Master System ROM only?
The CRC of the SMS and GG ROMs didn't match, so I rather have both. Plus people search for specific versions.
